Mohs Surgery

Micrographically controlled surgery for the most precise removal of skin cancer with maximum tissue preservation.

Mohs Surgery (Micrographic Surgery)

Mohs surgery, also known as micrographically controlled surgery, is the most precise method for removing skin cancer. In this technique, the tumor is removed layer by layer, and each layer is immediately examined under a microscope for remaining cancer cells. This process is repeated until the margins are completely tumor-free.

The decisive advantage of Mohs surgery lies in the complete histological control of the margins. Unlike conventional excision, where only random samples are examined, the Mohs technique evaluates 100% of the surgical margins microscopically. This achieves the highest possible cure rate – up to 99% for basal cell carcinomas.

Mohs surgery is particularly suitable for skin tumors in aesthetically or functionally sensitive areas such as the face, ears, nose, and eyelids. Through the layer-by-layer removal, only as much healthy tissue as absolutely necessary is removed, resulting in smaller defects and better cosmetic outcomes.

At our clinic, we combine Mohs surgery with state-of-the-art frozen section diagnosis and plastic-reconstructive techniques. This allows us to safely remove the tumor and aesthetically close the resulting defect in the same session.

Frequently Asked Questions about Mohs Surgery

Here you will find answers to the most important questions about micrographically controlled surgery.

Mohs surgery is a specialized procedure for removing skin cancer in which the tumor is removed layer by layer. Each layer is immediately examined under a microscope to ensure all cancer cells have been removed. This approach enables the highest cure rate while maximally preserving healthy tissue.
Mohs surgery is particularly suitable for basal cell carcinomas and squamous cell carcinomas, especially when located in the facial area, on the ears, nose, or eyelids. It is also the method of choice for recurrent tumors or tumors with unclear margins.
The duration depends on the size and depth of the tumor. Typically, the procedure takes between one and three hours, as each removed layer must be examined microscopically. In some cases, the procedure may take longer.
Mohs surgery is usually performed under local anesthesia. You are awake throughout the entire procedure but feel no pain in the surgical area.
Mohs surgery has the highest cure rate of all skin cancer removal procedures. For first-time basal cell carcinomas, the cure rate is up to 99%, and for recurrent tumors approximately 95%.
After the procedure, you will receive a wound dressing and detailed care instructions. The sutures are removed after approximately 7 to 14 days. Regular follow-up appointments are important to detect any recurrence early.
Depending on the size and location of the defect, various techniques are used: direct wound closure, local flap procedures, or skin grafts. As a specialist in Plastic Surgery, Dr. Caspari places special emphasis on achieving the best possible aesthetic result.
